MPH On-Campus Application
You can choose to complete the Department’s Application in either a fill-able PDF or a fill-able Word Document. NOTE: If you use the above, you MUST have Adobe Acrobat on your computer to save your data. If you use Adobe Reader you are able to fill in the forms but are not able to save it. If you only have Adobe Reader we recommend you use the application below–the Word fillable form.
Deadline is February 15.
You will need to include the following within your application:
Names of References
GRE Scores
Personal Statement
Curriculum Vitae/Resume
Copies of Unofficial Transcripts from at least one university (official transcripts need to be ordered and sent to NMSU, but for the application process, unofficial transcripts are acceptable)
